Interviewing and Patient-Reported Outcomes

When we hear the term “drug therapy monitoring” we may immediately think of
clinical monitoring using lab values or monitoring devices. However, monitoring
for many medications relies wholly or primarily on patient self-reports of therapeutic effect. Evaluation of depression, pain, anxiety, insomnia, migraine
headaches, and menopausal symptoms are based on patient self-report of symptoms. Management of other diverse conditions, including asthma, seizure control, arthritis, COPD, gastrointestinal disorders, and ADHD rely on patient or caregiver reports to diagnose the condition and monitor the effectiveness of therapy. Evaluation of tolerability or adverse effects of almost any drug therapy must
include self-report or caregiver report of symptoms.

In addition, the use of self-monitoring devices and patient communication of results is crucial in monitoring a number of chronic diseases, including diabetes, hypertension, and increasingly, anticoagulation therapy. Research has examined which patient-reported outcomes are used as efficacy endpoints in product labeling for drugs newly approved by the U.S. Food and Drug Administration (Willke et al, 2004). The patient self-reported endpoint measures
involved health-related quality of life, health status, event logs, symptom reports,
adherence reports, and satisfaction with treatment. Patient-reported outcomes
were endpoints in 30% of product labels reviewed. Some therapies rely exclusivelyon patient-reported outcomes to measure both effectiveness and adverse effects. Research has examined which patient-reported outcomes are used as efficacy

Pharmacists may, in such situations, be instrumental in conducting initial screening of such symptoms and referring the patient if depression seems probable.
